Social health clinic
Founded in 2021, JFC Social Health Clinic advocates for patient care beyond simply providing medical services. Interdisciplinary volunteer teams address social needs and create a holistic picture of a patient's overall health and wellbeing. Teams use social prescribing, or providing and referring individuals to community resources in an official capacity. Teams also "diagnose" social conditions and document them in the electronic health record to emphasize social determinants of health in other clinic visits. Resources provided include government assistance and insurance programs, sexual health products, medication assistance, housing assistance, financial strain resources, food products, transportation, and substance use programs, and interpersonal violence programs.

We understand the limitations of our clinic in meeting the needs of our patient population. Therefore, we have compiled a list of resources and partners that can provide low-cost services outside of our normal clinic hours, to supplement the care of the Jackson Free Clinic.
